I give Main Ohana Dance Studio permission to photograph and film my dancer during classes, rehearsals and performances, and to use those images in studio publicity — including our website, printed material and social media. No names are published alongside images of dancers. This permission can be withdrawn at any time by contacting the studio, and withdrawing it will not affect my dancer's participation in any way.

Liability release

I understand that dance involves physical activity and carries a risk of injury. I confirm that my dancer is physically able to take part, and I release Main Ohana Dance Studio, its teachers and its staff from liability for injury sustained during normal participation, except where caused by negligence. I agree to inform the studio of any medical condition that affects my dancer's participation, and to keep that information up to date.
